| On October 16, 2017, at approximately 3:30 PM, a cat-line was attached to a cable to raise up a rig floor. An employee was attempting to un-pin the floor to remove it when the cable snapped and the floor fell on the employee, striking him in the back and pinning him for approximately 10 seconds.
